FENDER TAG ?

Today I was going through some junk in a drawer and found a copy of a TEXAS title and a fender tag wrapped up in it. I DO have a pretty lousy memory, but for the life of me I cannot remember what this is from. I did pick up a 64' Dodge in Texas for a buddy, but the car was originally red and still was under the hood. This tag is green and looks very original. Also there is no reason I would have removed his tag.  So I am asking for your help. I have tried to find out for myself what this goes to without any luck. I am hoping one of you more experienced tag guys can help me out here..

I still have no idea how I ended up with this tag. I have never owned this car. At least I don't remember ever having one like it.. Maybe it was a car I stripped many years ago in NY. I crushed alllllllot of iron back then..
