Does anyone know what keeps the piece from going back in the hole. Some kind of inside star with a notch in it.

It's supposed to have a nut similar to what is on your ignition switch.

Doesn't that go on after the upper door pad? Now, when I try to put the upper door pad on it falls back in the hole. I thought there was a thin washer that held it. One with a notch in it

Yes Dub, this says 68 but my 67 has one as well.
