Questions on installing remote driver mirror?

Does anyone have a pic of how this goes together?  I am wondering if it is supposed to be such a tight fit trying to get the wand through the door.  Also, what does it fasten to, behind the door itself or behind the door panel with the chrome bezel fastening from the front?

I can't speak for '68s, but the '71s had a plastic retainer on the front edge of the door to hold the cable away from the window mechanism. When I replaced my driver's mirror, I just put a large wire tie through from the outside, wrapped it around the mirror cable and pulled it back out through the hole. Then back through itself and clipped off the extra. I don't know how the '68 attaches to the inner door.
