Mopar guy needs help decoding a build sheet...anyone?

A member of the Mopar Muscle Association UK has no Fender tag in his 1974 Dart Sport. He does have the remains of his build sheet. Can anyone make anything of this?
Any help greatfully received.

Oh well since I don't have anything else to do here goes Nick, btw since this is a 75 I have a lot of the option codes, just not all of them, my parts and casting numbers book just goes op to 1974.

Than a whole partion of the build sheet has to be decoded through part numbers which I don't have... Than we come at the most interesting part the option groups. But you are missing a large portion of it. I'll just decode what is readable.
