Help identify my Dad's old Dodge Polara.

My Dad left me his old, but nice, 1964 Dodge Polara. The Vin number is 6342166124 and the fender tag read's like this: Top row of #'s is blank, the next row of letters, under each letter is a # like:A=5, F=2, H=5, L=6, M=3, Q=1, R=4, T=4, W=2. And the next row down is abeverations and words with numbers under them, like this: SO=0120, NUMBER=0522, BDY=632, TRM=H4Y, PAINT=SS1.
